Privacy Notice

Federal and state laws require insurance agencies to provide customers with information related to privacy policies. Slater & Associates Insurance, Inc. is committed to protecting your personal information. The following policies describe how we protect your personal information.

Personal Information We Collect
Personal information we collect about you is used to provide and service insurance policies. This may include items related to your identity such as your name, address, and phone number. Depending upon the type of insurance you have, it may also include medical, financial, and driving information.
Personal information is gathered from your statements to us, applications, underwriting forms, consumer reporting agencies, and insurance companies.

Personal Information We Disclose About You
We do not disclose your personal information, except as required to perform insurance transactions as allowed by law. The law permits us to disclose personal information necessary to proceed with underwriting, claims, and other normal insurance functions (such as providing proof of insurance).

Nonaffiliated Third Parties To Whom Disclosures May Be Made
Your personal information is not disclosed to any nonaffiliated third parties, except as permitted by law.

Former Customers
We treat personal information in the same manner whether you are a current or former customer.

"Opt Outs"
Many companies provide you the right to “Opt Out” of having your personal information shared with others, usually in connection with marketing products. We do not make these kinds of disclosures, so it is not necessary for you to “Opt Out.”

Confidentiality and Security
Our company has safeguards that limit access to your personal information. We maintain physical, electronic, and procedural safeguards that comply with federal regulations in order to guard your personal information.

Our Policy Regarding Dispute Resolution
Any controversy or claim arising out of or relating to our privacy policy, or the breach thereof, shall be settled by arbitration in accordance with the rules of the American Arbitration Association, and judgment upon the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof.

We may revise our privacy policy as allowed by law, and will inform you of any such changes.
